REQUIRED PAPERWORK
Prior to any student participating in athletics, the following items must be on file with the Port Charlotte High School Athletic Department:- Physical Evaluation
Physical Evaluations are valid for 365 calendar days from the date of the evaluation as written on page 2 of the Physical Form - Copy of Medical/Health insurance card.
- Parent Consent
Parent Consent forms are valid until the end of the school year. - Team Policies and Regulations Agreement
Player and Parent(s)/Guardian(s) must read, agree, sign and return before participating in the Pirate Football Program.
